COMPLETING THIS FORM DOES NOT CONFIRM A BOOKING

Rentals begin on the hour, and each 60 minute rental includes a 50 minute skate followed by a 10 minute flood. 2 dressing rooms will be made available 30 minutes before and after each rental.

Payment in full and a signed rental contract are required before the booking can be confirmed.

If any aspect of this form is not completed truthfully, the event may be canceled.
